HOBAN Recruitment is recruiting for the role of APS4 level Planner on behalf of our client, located in DEAKIN ACT. This is a fantastic opportunity to contribute to a meaningful initiative that enhances capacity and capability within the disability support sector.
About the Role:
As an APS4 level Planner, you will be a key team member supporting the delivery of core initiatives and projects. You will play a crucial role in gathering participant information, making evidence-based funding decisions, and ensuring service delivery aligns with policy and practice standards.
Key responsibilities include:
- Gathering information from participants and planning partners via phone or in-person consultations.
- Applying policies, legislation, and practice guidance to inform funding decisions for participant plans.
- Working closely with participants and their representatives to identify current and future support needs.
- Liaising with key stakeholders, including Partners in the Community, to facilitate service delivery.
- Resolving issues and complaints related to participant plans.
- Managing administrative tasks and data entry related to participant services.
- Contributing to key performance indicators, including planning targets.
